What is Community Connect?
The Community Connect Program is an advocacy and support program. The aim of the program, is to provide assistance and support for vulnerable families and individuals, in our community.
The focus of the program is on persons with high needs including those affected by domestic and family violence.
The Community Connect worker will provide assistance through a case management framework that is client centred.

FREQUENTLY ASKED QUESTIONS
Am I Eligible?
The Community Connect Program is for vulnerable families or individuals with complex or high needs, including those affected by domestic and family violence.
The Community Connect Worker will work out your eligibility with you. If you are not eligible or unable to get into the service, other community centre staff can assist you with information and referral support.
Do I need a Referral?
NO—you can self-refer into the program by contacting the Community Connect Worker at the Emerald Neighbourhood Centre by phone on 4982 1696 or email at frontdesk@emeraldnc.com.au
If another service or worker has made a referral for you (with your permission) the Community Connect Worker will contact you to arrange a time to catch up.
Can I drop in?
YES—you can, but the worker may not be available. If you can, you should make an appointment or arrange a suitable time to speak to the worker.
How long can I access the service?
The length of time the Community Connect Worker will work with you depends on your level of need. It will be different for everyone…….
What Happens?
The Community Connect Worker will need to meet with you and this will happen here at the Emerald Neighbourhood Centre at an arranged time that suits you.
If transport or travel to attend an appointment is an issue for you, please speak to the worker to see if we can help. Together we will develop a plan to address your needs.
Emergencies?
The Community Connect Program is not a crisis service so is not available after hours.
